چکیده

OBJECTIVE To determine the configurations in pathologic audiograms obtained in patients shortly after exposure to an explosion. DESIGN, SETTING, AND PATIENTS Audiograms were performed in 143 patients (286 ears; 76 males and 67 females, with a mean age of 34.6 years [range, 11-79 years]) sent to the Department of Otolaryngology and Head and Neck Surgery in a city hospital located in the center of Jerusalem, Israel, after being injured in 4 severe explosions occurring in Jerusalem during 1995-1997. Most of the audiograms were obtained within 4 hours after the explosion, and the remaining were obtained within 4 days of the explosion. RESULTS Of 200 pathologic audiograms, 93 (46%) showed a downward slope configuration, 82 (41%) showed a dip configuration, and 25 (12%) were flat. There were 38 audiograms (19%) with 6-kHz dips. In 82% of the patients, the audiometric configurations were similar in both ears. Patients with a slope configuration on the audiogram were significantly older than those with a dip configuration (mean age, 40.8 vs 32.8 years; P<.01). CONCLUSIONS There does not appear to be a single typical audiometric configuration in patients exposed to explosions. The slope and dip configurations are most frequently seen and are approximately equal in their incidence. This observation may lead to better understanding damage to the inner ear as a result of an explosion. This study is distinctive because of the large number of audiograms obtained and the fact that most of them were obtained immediately after the explosion.

Audiometric notch as a sign of noise induced hearing loss.

OBJECTIVES To investigate the relation between different types of exposure to noise and a classic sign of noise induced hearing loss (NIHL), the audiometric notch. METHODS The study sample had exposure to both continuous and impulse noise and was drawn from a population of electrical transmission workers. Audiograms, taken as part of a hearing conservation programme, were read by three clinic...

Audiometric Configurations in Children

Recent studies suggest that the amplification needs of children and adults differ due to differences in perceptual ability. For example, several studies have confirmed that children realize greater benefit from a more audible speech signal (Stelmachowicz, Hoover, Lewis, Kortekaas and Pittman 2000; Stelmachowicz, Pittman, Hoover, Lewis and Moeller 2004), a higher signal-to-noise ratio (Hall and ...

In response to Determinants of bilateral audiometric notches in noise-induced hearing loss.

OBJECTIVES/HYPOTHESIS To explore the possible determinants of bilateral audiometric notches in noise-induced hearing loss (NIHL). STUDY DESIGN Prospectively enrolled cohort with cross-sectional analysis. METHODS Male workers (n = 412) with exposure to continuous loud noise (noise exposure level normalized to a nominal 8-hour working day >75 dBA) during work were included in this cohort stud...

Prevalence of Hearing Impairment by Gender and Audiometric Configuration: Results from the National Health and Nutrition Examination Survey (1999–2004) and the Keokuk County Rural Health

Purpose: This study describes the most common audiometric configurations and the prevalence of these configurations among adults (ages 20 to 69) in the noninstitutionalized population of the United States and in a sample of residents of a rural county in Iowa. Research Design: This was a cross-sectional population-based study. Study Sample: Estimates generalizing to the noninstitutionalized pop...

The physics, chemistry and dynamics of explosions.

The motivation for devoting a Theme Issue to explosions is discussed. As subsequent articles in the issue are written with the assumption that the reader has had a certain amount of previous exposure to the subject, some of the history and necessary background information are presented here. The topics on explosions that will be encountered in the remaining articles are previewed. Finally, seve...
